Attending Physician
The Attending Physician ensures stability and compassionate, person-centered care for patients by collaborating closely with the multidisciplinary hospice team. The physician remains in contact with the patient, providing continuous support and expertise throughout their end-of-life journey.

Hospice Medical Director
The Hospice Medical Director is responsible for certifying the prognosis given by the Attending Physician and coordinating with the hospice care team to develop a holistic, individualized plan of care that aligns with the patient’s preferences and needs.

Registered Nurse
The Registered Nurse plays a vital, practical role in patient care, conducting thorough evaluations during visits, implementing personalized care plans, and making informed decisions on pain management and symptom control to ensure the patient’s comfort and dignity.

Social Worker
The Social Worker assesses the social service needs of patients and their families, providing essential counseling, emotional support, and access to financial resources, thereby enhancing overall well-being during the hospice care experience.

Home Hospice Aide
The Home Hospice Aide offers direct, compassionate personal care and support to patients and their families, addressing daily needs and contributing to the holistic care approach that prioritizes comfort and dignity.

Chaplain
The Chaplain serves as a liaison with the clergy of the patientโs choice, offering spiritual guidance and support, and assisting with funeral and memorial preparations to honor the patient’s life and legacy.
