Hospice is an innovation in care built upon the concept of individual choice, allowing a patient with a life-limiting illness to stay at home surrounded by family, friends and a team of specially trained professionals and volunteers who work with the patient’s doctor to provide a plan of care designed with dignity of choice and the power of love.
Emotional, physical, and spiritual support services, combined with medical care and equipment, activity assistance, occupational therapy, speech pathology, dietary counseling, emotional support, bereavement counseling and medication management ensures that each patient has the highest quality of care.
The Angel Care Hospice & Palliative care program is a specialized approach to healthcare that focuses on improving the quality of life for our patients who have a serious or life-limiting illness.
Our goal is to relieve symptoms and stress that come with serious illnesses such as cancer, heart disease, and dementia.
Our Palliative Care team of healthcare professionals includes Doctors, Registered Nurses, Licensed Vocational Nurses, Social Workers, and Chaplains.
Palliative care is not the same as hospice care, although they share some similarities. While hospice care is typically reserved for patients who are in the final stages of their illness and have a limited life expectancy, palliative care can be provided at any stage of the illness, even if the patient is still receiving curative treatment.
The focus of our palliative care program is on managing symptoms such as pain, nausea, and fatigue, as well as addressing emotional, social, and spiritual concerns.
The goal is to improve the overall quality of life for our patients and their family, rather than focusing solely on the treatment of the illness.
Our Palliative Care Program can be provided in a variety of settings, including hospitals, Assisted Living Facilities, and the patient's home.
It is typically covered by health insurance, including Medicare and Medicaid, and may be provided alongside other forms of medical treatment. Our patients can continue to visit their Primary Care Physicians and Specialists. Please contact us for a no-cost evaluation.
