Registered Nurse

Hospice Medical Director

Attending Physician

Has the most practical and pragmatic role in the patient's care. During visits, the Registered Nurse evaluates the patient's condition, implements an individualized plan of care and decides on pain management.

Attending Physician

Hospice Medical Director

Attending Physician

In order to provide stability, the Physician for the patient stays with the patient and works in conjunction with the hospice team.

Hospice Medical Director

Is responsible for certifying the outlook/prognosis that is put forth by the attending Physician and coordinating with the hospice care team in developing a plan of care.

Chaplain

Home Hospice Aid

Hospice Medical Director

Works as a liaison with clergy of individual's choice. Provides spiritual support assistance with funeral and memorial preparations.

Home Hospice Aid

Provides direct personal care and support to the patient and family.

Social Worker

Home Hospice Aid

Executes an assessment of the patient and their families social service needs. The Social Worker also provides counseling and financial resources.
